What is an FCC Form 465 bid request?
USAC calls it the Description of Services Requested & Certification Form. The applicant lists the services it needs and publishes evaluation criteria so interested providers can prepare complete responses.
Once USAC approves the request, the Form 465 is posted publicly for at least 28 days. Providers review the official record and submit bids directly to the applicant, following the applicant’s stated requirements and deadline.

Map the HCP location and required endpoints to your service footprint. Identify whether delivery is on-net, near-net, partner-dependent, or construction-dependent. - 03

Can an applicant sign with a provider before the ACSD?
No. USAC states that an applicant must not agree to receive services or sign a contract before the Allowable Contract Selection Date. The official posting should be checked for the current ACSD and other deadlines.
